Room Request Form for Student Organizations & Interest Groups

Student Organizations and Interest Groups are required to complete a request form for room scheduling and video-conferencing between the two preclinical campuses.

The room request form includes the following information:

  • Student submitting the request
  • Date and type of event
  • Projected attendance (necessary to determine room size)
  • Request for video-conferencing or ZOOM (if applicable)
  • Origination campus for the video conferencing
  • Approval for IT on all participating campuses
  • Rooms reserved on all participating campuses
  • Ten-day advance schedule needed for meetings that require any kind of tech support
  • Contact persons on both campuses

If there is a request for video-conferencing or any type of technology, this request must be submitted and approved ten business days in advance.
